$1,500 Benefactor (16 guests)	Provides annual exam and birth control for 8 patients $940 is tax deductible
$1,000 Patron (12 guests)	Provides full STI screenings/treatment for 10 patients $580 is tax deductible
$700 Sustainer (8 guests)	Provides family planning "HOPE" visits for 10 patients $420 is tax deductible
$400 Sponsor (5 guests)	Provides one year oral contraception for 1 patient $225 is tax deductible
$100 Individual	Specify number of reservations on the next page $65 per reservation is tax deductible
$30 30 and under	Specify number of reservations on the next page non-tax deductible 2 free drink tickets per reservation, (additional drink tickets available for purchase with photo ID at $8/ticket)
